For Nursing Care Plans, How Would You Describe Skin Texture?

The “standard” answer when charting is PWD (pink, warm, and dry). This is what is expected. On your care plan, note anything that is an exception to this. Note if skin feels cool or warm to the touch, if there are petichiae, rashes, bumps, etc. Just describe what you observe and feel.
